When couples are trying to conceive, it is common for the focus to fall primarily on female investigations. However, male factor fertility contributes to around 40 to 50 percent of fertility challenges.

Understanding sperm health is an important and empowering step.

I offer Sperm DNA Fragmentation (SDF) testing in Edinburgh for men who would like a deeper understanding of their fertility, particularly if:

  • You have been trying to conceive for some time

  • You have experienced recurrent miscarriage

What is Sperm DNA Fragmentation?

A standard semen analysis looks at:

Sperm count
Motility (movement)
Morphology (shape)

While these are important, they do not assess the integrity of the genetic material inside the sperm.

Sperm DNA Fragmentation testing measures the level of DNA damage within the sperm cells themselves.

Even when count and motility appear normal, elevated DNA fragmentation may affect:

Natural conception
Embryo development
Miscarriage risk
IVF or ICSI outcomes

SDF testing provides another layer of information to help guide next steps.

What does a high SDF result mean?

A higher fragmentation result does not mean pregnancy is impossible.

It means there may be increased DNA damage within sperm, which can be influenced by factors such as:

Oxidative stress
Varicocele
Smoking
Illness or fever
Environmental exposures
Lifestyle factors

Results should always be interpreted in the context of your wider fertility picture and discussed with an appropriate clinician.

This test does not diagnose a specific cause, but it can help guide whether further medical review or lifestyle optimisation may be helpful.

Do I need a referral?

You can self-refer for the Exact® test. The Extend® and Extensiv® tests require referral from a clinician who can interpret results within your wider fertility pathway.


How is the sample produced?

Samples can be produced at home and delivered to the clinic within approximately 40 minutes, keeping the sample warm and transported carefully. Alternatively, samples can be produced at the clinic. Full preparation guidance is provided in advance.


How long does the appointment take?

Around 45 minutes.


How long do results take?

Usually within two weeks once the sample reaches the laboratory.


What if fragmentation levels are high?

A higher level means a greater proportion of sperm contain DNA damage. This does not mean pregnancy cannot happen. Results are always interpreted within clinical context.


Can Sperm DNA Fragmentation improve?

Yes. Sperm are renewed continuously and DNA quality can change, particularly where modifiable factors are addressed.


How is the test carried out?

The test involves providing a semen sample.

You will be given clear instructions beforehand, including:

• Recommended abstinence period
• How to collect the sample
• How and when to return it
• Timing for courier shipment

Semen samples should not be collected using standard condoms or lubricants unless specifically fertility safe.

Results are usually available within approximately two weeks.


Do I need SDF testing or a standard semen analysis first?

If you have never had a semen analysis, it is often recommended to start there.

A standard semen analysis assesses count, movement and morphology. SDF testing adds further information about DNA integrity.

In some cases, both tests are useful.
